Duo Health is a value-based medical group built around patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). Their model is different: they partner with health systems and insurers to identify CKD patients earlier, then provide holistic, home-based care while working alongside the patient’s existing doctors.

The mission is compelling. But the operational reality was painful.

Patient data arrived from insurance companies in messy, inconsistent formats. Duplicates were everywhere. Demographics were outdated. Insurance information was often invalid. Every patient record had to be manually deduplicated, validated, and corrected before anyone could start providing care.

Assigning patients to the right care team members? Also manual. There was no streamlined way to match patients with appropriate staff based on location, specialty, or capacity. The Operations and Experience teams spent hours on data cleanup that should have taken minutes.
